Formsite has been around since 1998 and it shows in the right ways: payments, workflows, PDF results and a long list of integrations. Pricing is flat and public, with no sales or discounts. The limits are the thing to check: the free level is barely a trial, and even paid plans cap results per form, so a busy registration form can outgrow Personal fast.
