It is free with any Microsoft account and included in Microsoft 365 work and school accounts. Free personal accounts are limited to 200 responses per form; Microsoft 365 business and education accounts allow up to 5,000,000.

Our take

Forms does the basics well: question types cover most surveys and quizzes, branching works, and results land in Excel. The free personal account is fine for small surveys but stops at 200 responses per form. Inside a Microsoft 365 organization the limit rises to 5,000,000, which is why most heavy use happens at work and school.

Why we rate it

It costs nothing extra for anyone with a Microsoft account, covers the common question types plus branching, and sends results to Excel. For Microsoft 365 organizations the response limit is effectively unlimited.

The catch

Free personal accounts stop at 200 responses per form and paid personal accounts at 1,000. Design and analysis options are limited; Microsoft points users who need more to Dynamics 365 Customer Voice.
