Try FormstackFrom $83/mo billed annually (Forms)

Formstack offers no-code software for online forms, document generation and eSignatures, and its maker renamed itself Intellistack in June 2025 while formstack.com still sells the Formstack products. Forms is $83 a month when paid annually ($99 on monthly billing) and Suite, which adds Documents and Sign, is $250 ($299 monthly); Salesforce Solutions and Enterprise are custom-priced, and a 14-day free trial is offered (formstack.com, read 4 November 2026). The visible prices carry a dollar sign without a currency code; the page's structured data lists them in USD.

Formstack's company is now Intellistack

A post dated 25 June 2025 on formstack.com says Intellistack, formerly Formstack, announced its rebrand. Formstack remains the name of the Forms, Documents and Sign products sold at formstack.com, and the post says both Formstack and Formsite products continue to be sold and supported.

Formstack's pricing page footer reads '(c) 2026 Intellistack, LLC'. Intellistack also sells a separate AI-powered workflow platform through intellistack.com, which this page does not cover.

What it really isNo-code forms, document generation and eSignature software sold as Formstack at formstack.com by Intellistack, the company's name since June 2025.
Our takeFormstack publishes its entry prices and security statements, though it words the trial differently on two pages.
The limits on Forms are tight, Suite costs about three times as much, and the larger plans are quoted, so the final bill depends on users, submissions and add-ons.
Why we rate itEntry prices, limits, the trial and security statements are published first-party, but the larger plans are quoted and extra users or submissions are not priced on the page.
The catchThere is no free plan, the trial is worded differently on two pages, and extra builder users, submissions or merges are priced after signup, not on the pricing page.

Best / not good for

Best for
  • Teams that need forms, documents and signatures from one vendor
  • Salesforce teams that want forms built natively for Salesforce
  • Organizations that want Formstack's published PCI and HIPAA plan statements
Not good for
  • Buyers who need a permanent free plan
  • Small teams that need more than 1 builder user or 25 forms on the entry plan
  • Buyers who want published prices for Salesforce Solutions or Enterprise
Trade-offs

Friction report

Trade-offs to know before signing up.

  1. Time to value
Formstack asks for no card during its 14-day trial, describes its builder as no-code, and runs an AI assistant around the clock.
  2. Scale breakpoint
Forms covers 1 builder user, 25 forms and 1,000 submissions per form; Suite covers 3 builder users, 100 forms and 5,000 submissions per form. Beyond that, Formstack says more users, submissions or merges can be added after signup.
  3. Walled garden
Document generation and eSignatures are part of Suite on the pricing page, and the comparison table marks HIPAA compliance as an add-on on Forms and Suite.

Frequently Asked Questions

How much does Formstack cost?

Formstack's pricing page shows Forms starting at $83 a month when paid annually ($99 a month on monthly billing) and Suite, which adds Documents and Sign, starting at $250 a month when paid annually ($299 monthly). Salesforce Solutions and Enterprise are custom-priced, and the page says annual plans save 2 months. The visible prices carry a dollar sign without a currency code, while the page's structured data lists them in USD, and Formstack takes Mastercard, Visa, Discover and American Express (checked 4 November 2026).

Is Formstack free?

No permanent free plan appears on Formstack's pricing page, but Formstack offers a 14-day trial, and no card is required to start it. The pricing page's FAQ describes the trial as covering the core Forms product, while the home page offers a 14-day trial of the full Suite (Forms, Documents, Sign and Workflows), so the two pages word it differently (checked 4 November 2026).

Who owns Formstack, and what is Intellistack?

Formstack's company is now called Intellistack: a post dated 25 June 2025 on formstack.com says 'Intellistack, formerly Formstack' announced its rebrand, and the pricing page footer reads '(c) 2026 Intellistack, LLC'. Intellistack's About page says it was founded in 2006 as Formstack, and Formstack remains the name of the Forms, Documents and Sign products, while Intellistack's AI-powered workflow platform is a separate product sold through intellistack.com. A December 2023 Formstack press release called the company PSG and Silversmith-backed.

Is Formstack safe to use?

Conversion Gems cannot certify any product as safe, but Formstack publishes its own security statements. Its data-security page says Formstack Forms is PCI compliant as a merchant and service provider, that HIPAA compliance applies to customers on its Forms HIPAA plan, that submission data is AES-256 encrypted at rest with TLS 1.2 or higher in transit, and that multi-factor authentication is optional. These are Formstack's claims, not our verdict.

Is Formsite the same as Formstack?

No, they are separate products. Formstack announced on 11 December 2023 that it had acquired Formsite, a Chicago-based forms provider founded in 1998, and its 25 June 2025 post says both Formstack and Formsite products continue to be sold and supported. Formsite is sold at formsite.com with its own pricing page.

How do I log in to Formstack?

Conversion Gems is a review site and cannot sign anyone in. The Sign In link on formstack.com opens https://admin.formstack.com/login, and the Help Center at help.formstack.com covers account questions. Intellistack's separate workflow platform has its own Login link on intellistack.com.
